Write the Approved Note Into the Chart

Who / when: the physician, at the moment you approve a scribe draft. This page is about what actually happens to your words once you sign them. There is no separate “scribe note” living off to the side — the moment you enter your eSign PIN (your 6-digit signing code) and select “Approve & sign into record”, the finished SOAP note is written into the patient’s encounter (the record of this one visit) as a normal, signed clinical note, identical in every way to one you would have typed by hand. Veona signs it with the central signing service, so it carries your signature, and it stamps on the provenance — which AI model drafted it and which speech-recognition engine transcribed it — so the record can always show how the note was made.

1. Physician — from the “Review draft” panel (on AI → Scribe, or the “Veona Scribe” bar on Chart → Scribe), confirm the SOAP fields read exactly as you intend, then enter your 6-digit eSign PIN and select “Approve & sign into record”.
2. Physician — Veona writes the four SOAP fields into the patient’s encounter as one signed clinical note, signs it under your name, and records the provenance with it.
3. Physician — read the confirmation. It tells you the note was saved to the encounter “with full provenance” and names the model that drafted it and the ASR (speech-recognition) engine that transcribed it — your evidence that the right note landed on the right visit, attributed and traceable.
4. Physician — open the patient’s notes view on Chart → Patients (open the patient) to see the new note sitting in the encounter, signed and dated, ready to read alongside the rest of the chart.

Note

The note is written to the canonical encounter — the one real record of the visit — not to a parallel scribe log. From this point it behaves like any other signed note: the orders, results, and bills on the same encounter all hang together on the one record.

Caution

Approving signs and locks the note. You can’t quietly edit it afterwards. To correct a signed note, use the eSign amendment chain — re-sign a corrected version with your PIN — which keeps the original intact and on the record. See Sign and Amend Any Record With Your eSign PIN.

What happens next

Your note is now part of the permanent chart and counts toward the encounter exactly like a typed note.
