Discharge Summaries
Discharge Summaries brings together the summaries from your ward and emergency department stays. Open one to read your follow-up instructions, download the summary, and book the recommended follow-up — all in one place.
What you’ll see
Section titled “What you’ll see”- A list of your stays, each showing the outcome, a reference number, the discharge date, and whether it was a ward or emergency visit.
- Clear tags when a follow-up is due or already booked.
Reading a summary
Section titled “Reading a summary”- Sign in at patients.veonahealth.com.
- Open Discharge Summaries from your portal home.
- Select a stay to open it. You’ll see your diagnosis and any follow-up instruction your care team recorded.
If a diagnosis is sensitive, it may be withheld here — the screen will let you know and suggest contacting the facility to review it.
Book your follow-up
Section titled “Book your follow-up”If a follow-up is recommended, select Book Follow-up. Your request is sent and your clinic confirms a time. If you’ve already booked, you can open that booking to confirm, reschedule or cancel.
Download the summary
Section titled “Download the summary”Where a summary document is available, select Download Summary to save a copy to keep or share with another doctor.
