Disclose and Deliver the Pack

Delivering is the moment the record actually leaves the facility — so it is also the moment Veona writes the disclosure. A records officer releases the fulfilled Medical Records Pack either by sharing it into the patient’s portal or by marking it for an in-person printed pickup. Both paths write one immutable row into the accounting of disclosures (the Disclosure Log): the permanent compliance record of who received what, over which scope, by which method, and when.

Who / when: the Records Officer, on a request that is Fulfilled (it has a compiled pack). (Delivery is a Records Officer capability.)

1. Records Officer — open Records → Request Queue, click the Fulfilled request to open its detail.
2. Records Officer — choose how to release it:
   • “Deliver To Portal” — shares the encrypted pack into the patient’s portal (the same visibility seam staff document-sharing uses) and fires a content-free notice to the patient: a heads-up with a portal link, never the records on the wire. (Use this when the patient is the recipient and has a portal.)
   • “Printed Pickup” — records an in-person collection of a printed copy. (Use this for a third party collecting in person, or a patient without a portal.)
3. In a single transaction Veona writes the immutable disclosure row — patient, requester type and name, the released categories, the scope window, the document count, the delivery method, who disclosed it and the timestamp — and moves the request to Delivered (a terminal state). A PHI-marked audit entry is recorded too.
4. Records Officer — for a portal delivery, the patient can now download the pack securely from their own portal session; for a printed pickup, print the pack and hand it over against the requester’s identity.

Note

The disclosure ledger is append-only by design. The application has no path to edit or delete a disclosure row — there is no update or delete on a disclosure anywhere in the system — so the accounting of disclosures cannot be altered after the fact.

Caution

Delivery is terminal: once delivered, a request cannot be re-opened or re-delivered. Choose the right delivery method, and for a printed pickup confirm the collector’s identity before you hand over the pack. The records pack itself never rides a notification — only the content-free portal notice does.

What happens next

The request is Delivered and its disclosure is now part of the immutable accounting of disclosures. A portal recipient downloads the pack from their own session; a pickup is handed over physically.
