Request Your Records From the Portal

Who / when: the Patient, whenever you want a copy of your own health record. The portal’s Records area lets you ask for your records, see where each request stands, and download a secure copy once it’s ready. This is your right of access — asking for your own records is free.

When you send a request, it doesn’t release anything automatically. A member of the clinic’s records team reviews every request and decides what can be shared, then prepares your records as a single secure file you download from this portal. Nothing is ever emailed to you — the file stays inside your authenticated session, encrypted until you ask for it back.

1. Patient — sign in to the patient portal (see Sign In, Reset and Recover a Portal Account).
2. Patient — open Records from the portal menu. You’ll see a “Request my records” form and, below it, a “My requests” list.
3. Patient — under “What records do you need?”, tick the kinds of record you want: visits, clinical notes, diagnoses, problems, lab results, imaging reports, prescriptions, vitals, or documents. Choose at least one.
4. Patient — set the From and To dates for the period your records should cover.
5. Patient — choose how to receive them: Download in this portal (the secure file appears here when ready) or Collect a printed copy at the clinic.
6. Patient — optionally add a reason, then press “Send request”. You’ll see “Your request was sent” and the request appears in My requests.
7. Patient — watch the status pill on your request. It moves from Submitted to Under Review while the team works on it, then to Approved (or Partially Approved), and finally to Delivered when your records are ready.
8. Patient — once a portal request is Delivered, a Download button appears on its row. Press it to get your Medical Records Pack — a single PDF with a cover page and a section for each kind of record — downloaded securely to your device.

Note

The clinic reviews every request and may approve only part of it — for example, releasing your lab results but holding back another category. Your request will show as Partially Approved, and the file you download contains exactly the categories that were approved. If a request is declined, its status shows the reason.

Tip

You’ll get a short notice (by your preferred channel) when your request is approved or your records are ready — but the notice never contains the records themselves. Always come back to the portal and press Download to get the actual file, safely inside your signed-in session.

What happens next

Your delivered records pack downloads straight to your device, exactly as compiled — no email, no link to a third-party site. Your file stays available to re-download from your My requests list.
