Trace A Unit Vein-To-Vein (Lookback)
When a transfusion goes wrong, the question is always the same: where did this blood come from, and where else has it gone? The answer has to be exact and it has to be fast — because if the unit traces back to a donor who turns out to be reactive, every other component made from that same donation may need pulling from a fridge before it reaches another patient. Lookback is the trace that answers it. It walks the unit’s full vein-to-vein graph in one read: donor → donation → unit → request and issue → patient → reaction. No spreadsheet, no phone calls down the supply line — one screen, end to end.
A word on the term. Vein-to-vein means the complete journey of blood from the donor’s vein, through processing and storage, to the recipient’s vein — and lookback is the act of reconstructing that journey from any point on it. Sibling components are the other products (red cells, plasma, platelets, cryoprecipitate) made from the same single donation.
Who / when: a Blood Bank Officer or Lab Scientist investigating a reported reaction or a flagged donor, with the treating Clinician consulted where the clinical picture matters — at the point an investigation is opened. Reading the trace is open to the blood-bank team; you launch it straight from the event that triggered the concern.
We will follow Ifeoma, a blood bank officer, who has just seen a moderate reaction land in the register and needs to know the unit’s full history.
Walk the chain
Section titled “Walk the chain”-
Blood Bank Officer — open Blood Bank → Reactions (the “Haemovigilance” register). Find the reaction you are investigating in the “Reaction Register” — Ifeoma spots the one that just came in, with its patient, unit and severity.

Blood Bank Officer — on that reaction’s row, press “Lookback”. The “Vein-To-Vein Lookback” panel opens, headed with the unit label, and resolves the unit’s whole graph from the reaction you started on.

Blood Bank Officer — read the chain top to bottom. Each step is a node on the unit’s journey:
- Donor — the donor’s name, their donor reference, and their current status (Active, Deferred or Permanently Deferred). This is the head of the chain.
- Donation — the donation this unit was produced from, by its reference.
- Unit — the unit label, its component (red cells, plasma, platelets, cryoprecipitate) and its blood group.
- Request → Issue — the transfusion request the unit was issued against and the issue record, with the time it was checked out of the cold chain.
- Patient — the recipient, and (for a completed transfusion) when it was transfused.
- Reaction — the reaction type and severity, closing the loop back to where you started.

Blood Bank Officer — with the donor and donation now in front of you, decide what the investigation needs. If the donor’s status or the reaction type raises a concern about the donation itself, the donation reference tells you exactly which batch of components to chase. Ifeoma notes the donation reference to check whether any sibling components are still in stock.

You are not piecing it together from separate screens — the trace assembles the entire vein-to-vein story in a single panel, which is precisely what an investigator needs to see in one glance.
Opened from a reported reaction
Section titled “Opened from a reported reaction”The ordinary entry point. A reaction is reported, it lands in the register, and the investigation begins by pressing “Lookback” on its row. Because the trace keys off the administration the reaction was filed against, the unit, the donor and the patient all resolve automatically — the reaction is the thread you pull, and the whole garment follows.
Using the trace to recall sibling components
Section titled “Using the trace to recall sibling components”- Blood Bank Officer — read the Donation reference off the lookback.
- Blood Bank Officer — go to Blood Bank → Inventory and find the other components produced from that same donation. If a donor is implicated, those siblings — red cells, plasma, platelets — may carry the same risk.
- Blood Bank Officer — quarantine, discard or recall them as the investigation requires, so a single bad donation cannot reach a second patient through one of its other components. As audit and haemovigilance evidence
Section titled “As audit and haemovigilance evidence”The lookback is read-only and reconstructs the chain from the records as they stand, so it doubles as investigation evidence. For a haemovigilance review or an audit, it shows — without anyone having to assemble it by hand — the complete, signed lineage of a unit: donor, donation, unit, issue, patient and outcome. It is the documented proof of where this blood came from and where it went.
What happens next
Section titled “What happens next”The trace turns a single reported reaction into a complete, defensible account of a unit’s life. From it, the blood bank can recall sibling components, flag or defer the donor if the donation is implicated, and hand an investigator a one-screen lineage that needs no reconstruction. This is the end of the vein-to-vein journey — the point at which every earlier step, from registering the donor to hanging the unit, can be proven to have happened in order, to the right people, with the right signatures.
