Clinical Assessments

What this is

Clinical Assessments is the scored-instrument engine. It lets a facility build reusable, standardised clinical questionnaires — a PHQ-9 depression screen, a Glasgow Coma Scale, a falls-risk score, a functional index — and then perform them on a patient with the scoring done for you. A clinician answers each item; Veona works out the weighted total itself and reads off the band that says what the total means (for example “5–9 = Mild”), so the same instrument is scored the same way every time, by everyone, with no arithmetic to slip on.

Key terms used on this page:

• Template (instrument) — a reusable scored questionnaire. It has a code, a name, a category (Physiotherapy, Neurology, Mental Health, Functional, Pain, Nutrition or General) and a sign-required flag.
• Parameter — one scored item (one question) on the template. Each has a type — Yes/No, a number within a scale, or a Select with scored options — and a weight (a multiplier, for instruments where some items count more than others).
• Score band — a min–max range of total scores that maps to an interpretation label — for example “5–9 = Mild.” When a performed assessment’s total lands in a band’s range, that band’s label is stamped on the saved assessment.
• eSign PIN — your 6-digit electronic signature, separate from your password. If a template requires signing, the clinician enters it to finalise the assessment; once signed, the assessment is read-only.

Who uses it

• Clinicians, nurses and physiotherapists perform assessments at the bedside — they can run an instrument on a patient and read the result, but they do not author templates.
• Administrators can also author and edit templates in the Instrument Library.

Editions

Clinical Assessments is an always-on base module — it needs no licence and is available in all editions. There is no module gate; access is governed purely by role, deny-by-default.

Screens

The work surface is two screens: the bedside entry and the admin builder. A patient’s assessment history is reached by performing or searching, never as a standalone menu entry.

Author a template (Instrument Library)

1. Open Assessments → Instrument Library and press New Instrument.
2. Fill the header: a Code (a short, stable handle — it cannot be changed later), a Name, a Category, and tick Sign Required if every performed assessment must be eSigned.
3. Add the parameters — one per question. For each, set the type (Yes/No, a number within a scale, or a Select with named scored options) and the weight (how much the item counts; leave at 1 for equal weighting).
4. Add the score bands — each a min–max range with an interpretation label (for example 0–4 Minimal, 5–9 Mild, 10–14 Moderate). The maximum is inclusive.
5. Save. The instrument becomes available to perform.

Perform an assessment

1. Open Assessments → Perform Assessment, choose the patient and the instrument.
2. Score every parameter. As you go, the total updates live and an interpretation badge shows the band the total falls into.
3. Press Record Assessment. Veona computes the weighted total itself — it never trusts a total typed in the browser — resolves the band whose range contains that total, and stamps the band label on the saved assessment.
4. If the template requires signing, enter your eSign PIN to finalise. Once signed, the assessment is read-only.

Note

Veona always does the scoring. The total is the sum of each item’s score × weight, and the band is resolved automatically — there is no manual arithmetic and no chance of a scoring slip.

Caution

A signed assessment is read-only — there is no edit. The score and band are a snapshot taken at perform time: editing a template later does not re-score historical assessments already saved against it. And there is no conditional or branching logic — every parameter on the template must be answered.
