Send eSigned Patient Instructions to the Portal

Who / when: a Clinician (or nurse), inside the EHR, who wants to send a patient clear written guidance — discharge advice, how to take a medicine, pre-procedure preparation — and the Patient, who reads it in the app. Unlike a back-and-forth message, an instruction is one-way and signed: the clinician puts their name to it with an eSign PIN, and the patient receives it to read and acknowledge.

A couple of terms:

  • Patient instruction — formal, one-way guidance a clinician sends to a patient. It is authored, signed, and then sent; the patient cannot reply to it (that is what secure messaging is for).
  • eSign PIN — a clinician’s 6-digit signing code, separate from the login password. Entering it is a legal signature: it confirms this clinician authored and stands behind this instruction.
  • Template library — a set of ready-made instruction templates the clinic maintains, so common guidance is consistent and quick to send.

We will follow Dr. Okafor, sending Amina her wound-care instructions after a procedure.

  1. Clinician — open the patient’s profile in Register → Patients (search for the patient, then open their record). On the profile, go to the “Notifications” tab — this is where patient instructions are authored and sent.
  2. Clinician — start a new instruction. Choose a template from the library if one fits (it pre-fills the title and body, which you can edit), or write your own. Set the instruction type and give it a clear title the patient will recognise.
  3. Clinician — write the guidance in plain language. The body supports light formatting; keep it patient-friendly.
  4. Clinician — when you are ready to send, enter your 6-digit eSign PIN. This signs the instruction in your name through Veona’s central signing service, exactly as you sign a note or a result.
  5. Clinician — send it. The instruction is now delivered to the patient, and the patient gets a content-free nudge on their usual channel — a “you have a new instruction” prompt with no clinical detail in the email or SMS itself.
  6. Patient — sign in to the patient app (see Sign In Passwordless With a Magic Link) and open the Care tab: “Instructions and guidance your care team has sent you.” New items carry a New badge.
  7. Patient — press “Open” on the instruction. It opens with its type, the date it was sent and the facility’s name, and the full guidance. Opening it marks it as read.

The patient keeps every instruction on their Care tab to re-read whenever they need it, and the clinic can see that it was delivered and read. If the clinician later corrects an instruction, the patient’s copy updates and is marked as such. For a conversation rather than a directive, the patient can open the Messages tab and write to the care team.
