Pre-Visit Intake

Pre-Visit Intake is where you complete the forms and consents your clinic needs before your visit. Filling them in ahead of time makes your visit quicker and smoother.

What you’ll see

  • A list of forms and consents assigned to you, each showing what it is and which visit it’s for.
  • A clear marker for anything that’s required and still to be done, and whether each item is pending or complete.

Completing a form

  1. Sign in at patients.veonahealth.com.
  2. Open Intake from your portal home.
  3. Select an item to open it.
  4. Answer the questions, then select Submit Intake — or Save Draft to finish later. Your answers are kept, so you can pick up where you left off.

For a consent, read the wording, tick the box to say you agree, type your full name to sign, and select Sign Consent. You’ll get a confirmation with a consent number for your records.

If there’s nothing to complete, the screen will let you know — you’re all set for your visit.
