Read the Disclosure Log — Accounting of Disclosures
Every time a record leaves the facility, Veona writes it down — permanently. The Disclosure Log is the facility’s accounting of disclosures: an append-only ledger of every release of information, recording who received what, over which date range, by which delivery method, and when. It is the evidence a compliance officer or a patient can rely on to answer “who has seen my record?”.
Who / when: the Records Officer (or a compliance reviewer), whenever they need to see what has been disclosed — for an audit, a subject-access query, or a routine compliance check. (Reading the log is a Records Officer capability.)
- Records Officer — open Records → Disclosure Log.
- Records Officer — read each disclosure row: when it was disclosed, who it was disclosed to (requester name and type), the record categories released, the scope window, the delivery method (Portal or Printed Pickup), and the document count.
- Records Officer — to trace a disclosure back to its request, open the matching request from Records → Request Queue; the request detail also lists the disclosures raised against it.
What happens next
Section titled “What happens next”The disclosure ledger is retained as the facility’s compliance record. There is nothing more to do on a delivered disclosure — it stands as written. New disclosures appear here automatically as records officers deliver fulfilled requests; see Disclose and Deliver the Pack.
