Capture a Note From Inside the Chart

Who / when: the physician, while you have the patient’s chart open and would rather not switch screens to dictate. Veona puts the scribe right where you’re working: the “Veona Scribe” bar on the chart. It isn’t a second, lesser scribe — there is only one scribe pipeline in Veona, and the in-chart bar drives that same pipeline end to end: capture the audio, run the ASR (Automatic Speech Recognition, the self-hosted engine that turns speech into text) and the clinical model, review the SOAP draft (Subjective, Objective, Assessment, Plan), and sign it into the encounter with your eSign PIN (your 6-digit signing code). The session is “locked” globally, which means it stays alive as you click around the chart — you can look something up mid-dictation and the scribe won’t lose its place.

We’ll follow Dr. Adeyemi, documenting without leaving the chart.

1. Physician — open the patient on Chart → Patients, then open Chart → Scribe (the “Veona Scribe” entry point) and select “Start Scribe”. You’re already on the right patient and encounter, so there’s nothing to look up.
2. Physician — while writing a note you can also reach it from the notes view: select “Use Veona Scribe” instead of typing, and dictate the note rather than keying it.
3. Physician — capture the consultation. Select “Start recording” for a live consult, or “Upload audio” for a clip you already have, then “Generate draft”. The job goes onto the background queue — exactly the same path described in Turn Dictated Audio Into a SOAP Draft.
4. Physician — keep working in the chart while it transcribes and drafts. Because the session is locked globally, navigating to another lens of the chart won’t drop it.
5. Physician — when the status reads “Draft ready for review”, read and correct each SOAP field, then enter your 6-digit eSign PIN and select “Approve & sign into record” — the same human-in-the-loop gate covered in Review, Edit and Approve the Scribe Draft.

Note

Whether you start from AI → Scribe or from the chart’s “Veona Scribe” bar, it is the same engine, the same draft, and the same signed note at the end. The in-chart bar simply saves you the trip — there is no separate in-chart scribe with different behaviour.

Tip

Because the session survives navigation, you can dictate, then jump to the patient’s results or problems to check something, then come back to review the draft — without starting over.

What happens next

On approval the note is written into the patient’s encounter as a signed clinical note, with the model and ASR provenance recorded — see Write the Approved Note Into the Chart.
