Perform a Scored Assessment at the Bedside

Who / when: a clinician or physiotherapist (a nurse may also perform) scores a standardised assessment on a patient whenever a recognised measure is needed — a screening at first visit, a re-measure to track progress. Once an instrument has been built, performing it is quick: pick the patient and the instrument, answer each item, and Veona scores it for you and tells you which band the total falls in.

Perform the assessment

1. Clinician — open Assessments → Perform Assessment.
2. Choose the Patient (search by name or UHID — the unique hospital identifier) and the Instrument. The scoring table appears with one row per parameter.
3. Answer each parameter. Depending on the item, the input is a Yes/No toggle, a number within its scale, or a Select of scored options. Every parameter must be answered — there are no optional or skipped items.
4. As you score, the Total updates live and an Interpretation badge shows the band the total falls into. The Record Assessment button becomes available once every parameter has a score.
5. Press Record Assessment.

Note

Veona computes the weighted total itself — the sum of each item’s score × weight — and resolves the band whose range contains that total. It never trusts a total typed in the browser, so there is no arithmetic to get wrong, and the band that is saved is the right one for the score.

Sign it, if the instrument requires it

If the instrument was built with Sign Required on, the recorded assessment must be finalised with a signature. Press Sign, enter your 6-digit eSign PIN (your electronic signature, separate from your password), and the assessment is locked — your signature attests the score.

Once signed, the assessment is read-only: there is no edit. Instruments that do not require signing are saved as soon as you press Record Assessment.

What happens next

The saved assessment carries its total, its band label and who performed it, as a snapshot from this moment. Because every performance of the same instrument lands on the same patient record, repeat scores line up over time — a falling pain index, a rising functional score — so you can show concrete evidence of progress, scored consistently every time.
